A large leap in laser technology occurred several years ago with the advent of what is known as “Fractional Resurfacing.” Many different laser devices are known as fractional resurfacing devices which can be confusing to those wanting to know about these procedures in differentiating which can be used for fine lines and wrinkles, for acne scarring or for stretch marks.

Fractional resurfacing devices are typically divided into two kinds – non-ablative and ablative. Both devices work by creating small holes in the skin with “skip” areas of normal skin next to the holes which allow for faster, and safer healing of the skin.

1. 1.      Non-ablative fractional resurfacing devices do not affect the top layer of the skin in that the light penetrates through the epidermis and makes its holes in the deeper layers of the skin only. This results in less downtime and faster healing. These non-ablative therapies work over time and usually require a series of up to 4-6 treatments, spaced at about 3-4 weeks between therapies, to be successful.
2. 2.      Ablative fractional devices, on the other hand, penetrate through the epidermis and dermis to create their “holes” or fractional affects. They can result in more downtime but usually only require 1-2 treatments to exert their positive effects..

Affirm MPX laser is an advanced non-ablative fractional device.  Dr. Saluja actually evaluated this device during her dermatologic cosmetic surgery fellowship in La Jolla, CA – performing research with her preceptor, Dr. Mitchel Goldman, which has been published in the dermatology literature and presented at the American Society of Laser Medicine in Surgery Conference.   The attributes of this non ablative fractional laser makes it known worldwide as an efficacious laser.

Laser treatments are taken to the next level with the Combined Apex Pulse (CAP) technology found in the Cynosure Affirm™ MPX system. The Cynosure Affirm is a non-ablative fractional light energy device that allows for an effective treatment when dealing with acne scars, wrinkles and texture and tone concerns. The CAP system utilizes an array of specially designed lights which can penetrate the skin in a fractional method to achieve its desired effects.

The Cynosure Affirm MPX laser can be used as well to treat fine lines and wrinkles, age spots, sun-damaged skin and improve skin texture. A topical anesthetic is usually applied prior to treatment, and cool air is utilized as well which makes the treatment comfortable and tolerable to all.

The Cynosure Affirm MPX laser utilizes a wavelength of light at 1440 nm which targets the water in the skin. A second wavelength, at 1320 nm, has been shown to be beneficial for skin tightening. Thus the Cynosure Affirm MPX allows for the improvement in your lines and wrinkles and scars, and provides tissue tightening as well – all in one device. Because it is non-ablative, downtime is minimal with most patients being red for only up to one day or so following the treatment. The number of treatments that are needed will also vary, but because it is non-ablative, most require 4-6 treatments for optimal results.

• All skin types can be treated effectively with very little to no downtime post-treatment. The wavelength of light emitted by this device is color-blind – so all skin types can be safely treated. The Affirm works by insertingmicro-pulses of light to the skin’s dermis to aid in collagen stimulation, correction of wrinkles, acne scars, and sun damage. Common side effects are mild redness that usually resolves in just a few hours. A less common side effect is mild puffiness and swelling that quickly resolves. Post-inflammatory hypo- or hyperpigmentation, commonly seen with many laser devices, especially in darker skin types, is kept to a minimum with this device. Scarring, although rare with non-ablative fractional devices, can be seen as well.

We recommend a series of 4-6 treatment sessions depending on the type and severity of the condition being treated. Many patients state they are experiencing overall skin tightness and clarity with their first treatment.
